Overview
Telmisartan is a long-acting angiotensin II receptor blocker (ARB) indicated for the treatment of hypertension and for cardiovascular risk reduction in patients unable to take ACE inhibitors.

Mechanism of Action
It selectively blocks the binding of angiotensin II to the AT1 receptor, leading to vasodilation and a reduction in aldosterone secretion, which lowers blood pressure.

Dosage and Administration
The usual starting dose is 40 mg once daily, which may be increased to a maximum of 80 mg once daily depending on blood pressure response.

Side Effects
Generally well-tolerated. Potential side effects include dizziness, back pain, and sinusitis.

Contraindications
Contraindicated during pregnancy and in patients with severe hepatic impairment or biliary obstructive disorders.

Pregnancy and Lactation
Use is contraindicated during the second and third trimesters of pregnancy. Not recommended during breastfeeding.
