Overview
Fenofibrate is a fibric acid derivative used primarily to treat severe hypertriglyceridemia and mixed dyslipidemia. It helps lower triglycerides and increase HDL cholesterol.

Mechanism of Action
It activates peroxisome proliferator-activated receptor alpha (PPARα), which increases lipolysis and elimination of triglyceride-rich particles from plasma, and increases the synthesis of HDL.

Dosage and Administration
Dosage varies by formulation (e.g., 145 mg or 160 mg daily). It should generally be taken with meals to optimize absorption, depending on the specific product.

Side Effects
Common adverse effects include abnormal liver function tests, abdominal pain, and back pain. It can increase the risk of gallstones and myopathy, especially when combined with statins.

Contraindications
Contraindicated in patients with severe renal impairment, active liver disease, gallbladder disease, or nursing mothers.

Pregnancy and Lactation
Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It should not be used in nursing mothers.
