Heart Specialist Recommends How Diabetics Should Manage Blood Lipids

High blood lipids and high blood sugar are two major risk factors for cardiovascular disease. However, they often go unnoticed, with some patients only discovering abnormal indicators when complications arise. Specialist doctors remind the public to undergo regular blood lipid and blood sugar tests, and to intervene early if levels are found to be excessively high, to prevent irreversible vascular damage.

High blood lipids and high blood sugar attack, cardiovascular health is precarious
Dr Fu Chiu-lai, a cardiologist, explained that high cholesterol and diabetes are very closely linked, often appearing together and causing each other. Latest data shows that approximately 30-40% of diabetic patients die from cardiovascular disease. Type 2 diabetes patients experience insulin resistance, which prevents cells from effectively using glucose through insulin. As a result, sugar remains in the blood, becoming raw material for producing cholesterol and triglycerides, leading to lipid problems. High lipids, in turn, negatively affect the liver cells' response to insulin, creating a vicious cycle.

High blood lipids and diabetes can both cause inflammation, hardening, narrowing, and even blockage of blood vessels, leading to fatal diseases such as myocardial infarction, stroke, and kidney failure. If a patient suffers from both, their risk will be even higher. However, the symptoms of these two conditions may not be obvious. When blood vessels begin to be damaged, patients may still think they are fine, only to discover that their blood lipids and blood sugar levels have spiked when they are admitted to hospital due to an acute event. Therefore, Dr. Fu recommends that individuals over 40 should have a blood test annually for early detection of abnormalities. If there is a family history of cardiovascular disease, diabetes, or high blood pressure, earlier screening may be necessary.

Cholesterol targets are stricter for diabetics
To reduce the risk of cardiovascular disease, diabetics not only need to manage their blood sugar properly but also strictly control their blood lipid levels. Taking "bad" cholesterol as an example, the general public is considered to have met the standard if they achieve 3.0 mmol/L or below. However, patients diagnosed with diabetes for ten years need to control it to 1.8 mmol/L or below to meet the target. If the patient has already experienced vascular occlusion or organ complications, the value may need to be even lower. How do diabetic patients manage their blood lipids? Dr. Fu stated that developing good dietary and exercise habits is fundamental. In terms of diet, saturated fats, trans fats, and cholesterol intake should be reduced, and replaced with foods rich in unsaturated fats and high dietary fibre. In terms of exercise, at least 120-150 minutes of moderate-intensity aerobic exercise and appropriate muscle strengthening exercises should be performed weekly.

Drug therapy prevents "heart troubles"」
However, many people with diabetes still have suboptimal blood lipid levels even if they lead a healthy lifestyle and have no unhealthy habits; some patients also have a risk of developing cardiovascular disease within ten years of 10% or higher. They all need to consider medication to prevent cardiovascular disease from developing.

Currently, there are several types of cholesterol-lowering medications in Hong Kong that reduce cholesterol synthesis and absorption by targeting the liver and intestines, thereby lowering blood cholesterol levels. Traditionally, oral statins have been the first-line treatment, with studies proving their efficacy in reducing "bad" cholesterol and triglyceride levels. If further reduction of "bad" cholesterol is required, other medications can be added according to the patient's individual needs, such as oral cholesterol absorption inhibitors or injectable PCSK9 inhibitors.

Please note: The above information is for reference only. There are various treatment options, each with different efficacy, side effects, and risks. Patients should discuss with their doctor and choose the most suitable treatment plan based on their individual circumstances, including their physical condition, illness, and affordability.

ref:
